Collageville Order Form

1)   To place an order just fill in the form below.

2)   E-mail us your images using the e-mail link at the bottom of the page.

3)   Click submit to review your information.

4)   Click submit once more and you'll see the 'thank you' page.

When you place an order you'll receive an automated e-mail acknowledging your order.

When we confirm that your images have been received we'll send another e-mail and a PayPal payment request.

Work begins on orders when PayPal informs us that the payment has been received, or is pending.
